The Health Center provides a new approach to co-ordinating care. It has been designed to deliver an efficient and effective way for residents, their families and carers to access health and social care services. Working in partnership with health and social care organisations across health center to ensure people can get the right care, at the right time, in the right place.
The health center delivers a streamlined referral process, that aims to get people to their first appointment at the hospital with all relevant tests and information in place. This helps the first meeting with the consultant to be a decision-making appointment, reducing the need for return visits.
The Healthcare Co-ordination Centre is also a central point of contact which co-ordinates health and social care for people with multiple complex needs.
The whole ethos of the service is to equip people to manage their healthcare, keep them comfortable at home and offer non-medical options where appropriate.
The Reporting Data analyst will be part of the Performance Management Team and will be expected to report to the Performance Manager daily.
The primary function will be to meet the operational reporting requirements for the Health Center i.e. to produce reports required to support operational teams with meeting their SLAs, primarily using Tableau, MS Dynamics CRM standard reporting capability and MS Excel.
Over and above meeting the day to day reporting there is the opportunity to help change and shape the reporting solution as well as introducing new tools to automate and streamline the BAU function.
The Performance Team also plays a huge role in working with the technical teams to improve the software (Dynamics CRM) and equally with the users to streamline operational processes.
This is a great opportunity for an individual with an interest in Big Data and Analytics. The project is a first of type in the country and is ambitious in what it wants to achieve. Data from various NHS organisations provide for a rich integrated patient record. The service aims to obtain insights from the various data sources to help improve patient outcomes. The right candidate will be instrumental in delivering this vision.
